GST compliance is one of those things that feels complicated until someone explains it clearly. For dental clinics in India, the rules are actually straightforward but your billing software needs to handle them correctly or you will end up with invoices that do not hold up under scrutiny.

Here is everything you need to know.


Are dental services taxable under GST?

Most dental healthcare services are exempt from GST. Under the GST Act, services provided by a clinical establishment in the course of diagnosis, treatment, or care are exempt. This covers the core of what most dental clinics do: consultations, fillings, root canals, extractions, scaling, and similar treatments.

However, cosmetic dental procedures that are not medically necessary can attract GST at 18 percent. This includes teeth whitening, cosmetic veneers, and similar aesthetic treatments.

Dental products sold by the clinic, such as whitening kits, mouthguards, and retainers, are sold as a product rather than a service also attract GST.

The practical implication: most of your invoices will have zero GST, but some line items may be taxable. Your software needs to handle both on the same invoice cleanly.


What a GST-compliant dental invoice must include

Whether or not GST applies to the services, if your clinic is GST registered, your invoices must meet certain requirements. A compliant invoice must include:

  • Your clinic name and address

  • Your GSTIN

  • Invoice number (sequential, no gaps)

  • Invoice date

  • Patient name

  • Description of each service or product

  • HSN or SAC code for each line item

  • Amount before tax

  • GST rate and amount (or explicit note that the service is exempt)

  • Total amount payable

Missing any of these fields makes the invoice non-compliant. This matters if your patient wants to claim ITC or if you are ever audited.


What is the correct SAC code for dental services?

Dental healthcare services fall under SAC code 999312. Dental and orthodontic services. This code covers the full range of clinical dental treatments.

For cosmetic procedures, some practitioners use the same code since the distinction is the nature of the procedure, not a separate code. Consult your CA if you have a significant volume of cosmetic treatments.


What to look for in billing software

A lot of generic billing software handles GST for product businesses well but falls short for healthcare. Here is what a dental-specific billing tool must do:

Handle exempt and taxable line items together. You should be able to add a root canal (exempt) and a whitening treatment (18% GST) on the same invoice and have the software calculate correctly.

Generate a proper invoice PDF. The PDF must include all the required fields listed above. It should look professional enough to hand to a patient or submit to an insurer.

Sequential invoice numbering. GST rules require invoice numbers to be sequential with no gaps in a financial year. Your software must enforce this automatically manual numbering leads to errors.

GSTIN field for the clinic. Basic, but some generic tools skip this. Your clinic's GSTIN must appear on every invoice.

Patient-facing sharing. Being able to share an invoice link via WhatsApp so the patient can view and download their invoice reduces front-desk friction significantly.


What about clinics that are not GST registered?

If your annual turnover is below the GST registration threshold (currently ₹20 lakh for services), you are not required to register or charge GST. In this case, your invoices do not need a GSTIN, but you should still issue proper receipts with your clinic name, date, patient name, and service description.

As your clinic grows past the threshold, registration becomes mandatory. It is better to set up your billing software correctly from the start rather than migrating later.
